Background information
In 2023, about half of the Swiss population (48%) lived with at least one chronic disease , with most of them following long-term medication regimens. Optimizing outcomes for chronic disease patients remains a key priority in the Swiss Federal Council's health strategy. To do so, frameworks focusing on integrated ambulatory..
